GST on therapy sessions in India

Published Updated

Do therapists in India charge GST on sessions?

Often not: GST exempts health care services by a clinical establishment, an authorised medical practitioner or para-medics, and someone who supplies only exempt services does not have to register. But the law does not say clearly whether every counsellor, psychologist or allied therapist falls inside that exemption, one advance ruling taxed some therapy services at 18%, and wellness or coaching work is taxable. Check your own position with your CA.

The health care exemption

Entry 74 of Notification 12/2017-Central Tax (Rate) sets a nil rate for "health care services by a clinical establishment, an authorised medical practitioner or para-medics". The notification defines the key words:

  • Health care services: "any service by way of diagnosis or treatment or care for illness, injury, deformity, abnormality or pregnancy in any recognised system of medicines in India".
  • Authorised medical practitioner: "a medical practitioner registered with any of the councils of the recognised system of medicines established or recognised by law in India", including a professional with the qualification to practise under any law in force.
  • Clinical establishment: "a hospital, nursing home, clinic, sanatorium or any other institution by, whatever name called, that offers services or facilities requiring diagnosis or treatment or care for illness".

"Para-medics" is not defined in the notification, and GST law does not define "recognised system of medicines". That is where the uncertainty for therapists comes from.

A second entry, 74A, added from 1 January 2019, exempts services by rehabilitation professionals recognised under the Rehabilitation Council of India Act, 1992 "by way of rehabilitation, therapy or counselling", but only at medical establishments, educational institutions and rehabilitation centres set up by government or by charitable entities registered under section 12AA or 12AB. A private practice is outside that condition. The September 2025 rate changes did not alter entries 74 or 74A.

Where it is unclear

  • In May 2025 the Kerala Authority for Advance Ruling (KER/14/2025, Radiantminds Rehab LLP) held that services of occupational therapists, speech therapists and psychologists were not exempt under entry 74A and were taxable at 18% (as reported by StudyCafe; we could not find the ruling itself).
  • In March 2020 the West Bengal Authority for Advance Ruling held that a physiotherapy centre offering "phytotherapy" had not shown its treatment was in a recognised system of medicine, so entry 74 did not apply (order 46/WBAAR/2019-20).
  • We found no ruling directly on a solo counsellor, psychotherapist or life coach. Advance rulings bind only the business that asked for them, but they show how authorities read the law.

If a service is not exempt, therapy and counselling usually sit under heading 9993, human health and social care services, at 18% (9% central tax and 9% state tax) under Notification 11/2017-Central Tax (Rate). Coaching, corporate workshops and wellness programmes are taxable services too; ask your CA which heading and rate applies.

Do you need to register?

Under section 23(1)(a) of the CGST Act, "any person engaged exclusively in the business of supplying goods or services or both that are not liable to tax or wholly exempt from tax" is not liable to register. If any of your work is taxable, section 22(1) requires registration once your aggregate turnover in a financial year exceeds twenty lakh rupees, or ten lakh rupees in the special category states. Aggregate turnover includes exempt supplies (section 2(6)), so exempt session fees count toward the limit once you also earn taxable income such as workshop fees.

A registered practitioner who supplies exempt services issues a bill of supply for them instead of a tax invoice (section 31(3)(c)). Our guide to invoices and payments covers what each document needs.

How Bliss helps

Choose the GST treatment once for your clinic. Invoices carry your clinic's name and GSTIN, follow that setting, and show your SAC code and the CGST, SGST and IGST breakdown when GST is on. Bliss does not decide whether your sessions are exempt; that is a question for your CA.

Record cash, UPI and bank transfers against each invoice, and see what is still due.

The GST summary lays out your invoices and credit notes for a month or quarter as the tables of GSTR-1, with files for the GST offline tool. Bliss files nothing, so check the figures with your CA before you upload.

Questions people ask

Is counselling exempt from GST in India?
It can be, when it is a health care service by an authorised medical practitioner, para-medic or clinical establishment. The law does not define para-medics, so whether a particular counsellor qualifies is not settled. Ask your CA and keep a note of the reasoning you rely on.
Do I need a GSTIN if I only see therapy clients?
Not if every service you supply is exempt: section 23(1)(a) of the CGST Act says a person supplying only exempt services is not liable to register. If some of your work is taxable, the turnover limit applies to your total, including exempt fees.
Is physiotherapy exempt from GST?
The notification does not define para-medics, so it does not say outright whether a physiotherapist is one. In one advance ruling, a physiotherapy centre was refused the exemption for a treatment it had not shown to be part of a recognised system of medicine. Check with your CA.
What GST rate applies if my sessions are taxable?
Usually 18% under heading 9993, human health and social care services. Your CA can confirm the heading for your exact services.
