Diagnoses (ICD-10-CM)

Add a diagnosis by searching ICD-10-CM codes and titles, or add a custom one.

  1. Open the client and choose the Plan tab.
  2. Under Diagnoses, use Add a diagnosis: search by code or words, like F41.1 or panic. Common codes show before you type.
  3. Pick the match. It is saved with its official title.
  • For a code not in the list, choose Add a custom diagnosis and type the Code and Diagnosis name. It shows as "Custom or retired code".
  • Search shows the first 20 matches; type more to narrow it.
  • Codes and titles are ICD-10-CM FY2027, published by the National Center for Health Statistics (CDC), the US clinical version of the WHO's ICD-10.
  • The client's therapist, owners and admins add diagnoses; others who can read the record see them.
